Community Care Partners

Vancouver Island Health Authority — Home & Community Care

The Vancouver Island Health Authority (VIHA) is committed to supporting those who face end-of-life challenges. Especially important is the partnership that VIHA shares locally with Victoria Hospice in providing community-based palliative care to people in the comfort of their own homes.

VIHA Home & Community Care Nurses are usually the first contact for people needing to register for all Victoria Hospice services.  A phone call to the VIHA Health Unit located in your neighbourhood begins the process of contact and registration with Home & Community Care (HCC) and Victoria Hospice.

Sooke Hospice

In the mid-1980’s, a small group of volunteers in Sooke, a scenic seaside community about a 45 minute drive west of Victoria developed a hospice program that considered the special needs of the Sooke population, and the value that in-home support would have for people reluctant to leave their community in order to receive end-of-life care in a hospital or other facility far from home.

The program of care that Sooke Hospice has developed remains uniquely appropriate to its community’s needs. The team of Home Care Nurses, physicians, volunteers, Community Health Workers, and counsellors works together to assess and fulfill the needs of Sooke patients and family members.

Volunteer training for Sooke is provided by Sooke Hospice in the community.

For more information, telephone (250) 642-4345 or visit the Sooke Hospice website.

Saanich Peninsula Hospital

Saanich Peninsula Hospital has a 10-bed palliative care unit for patients living on the Saanich Peninsula north of Victoria who require short-term symptom management or a longer stay of up to three months.  This service is open to all residents within the Vancouver Island Health Authority - South Island region.

Access to the Saanich Peninsula Palliative Care Program is primarily through the family physician, who can refer to community resources and Victoria Hospice services, as well as mobilize services within a facility, such as the Saanich Peninsula Hospital, Sidney Intermediate Care and Rest Haven Lodge.

For more information on the Saanich Peninsula Hospital Palliative Care Program, please call their Clinical Care Coordinator at (250) 361-5429, 7 a.m. to 7 p.m. Pacific Standard Time.
